Patient Experience Surveys

Patient, family member and supporter feedback is important to us as it helps us plan and improve services. Ontario Shores is conducting Ontario Perception of Care Tool for Mental Health and Addictions (OPOC-MHA) surveys for patients and their family members/supporters. Please encourage patients, families and substitute decision makers to complete the survey anytime during their care, or shortly after discharge.

This survey questionnaire is anonymous and therefore all data collected will not be identified as your own responses. Your responses will be combined with responses from many other people and used for quality improvement purposes. Please see the Client Information Letter for more information about the survey.

How to obtain copies
To obtain printed copies of the OPOC-MHA patient or family member/supporter experience surveys please contact the Quality/Patient Safety/Patient Experience office at 905.430.4055 extensions 6770, 6769, or 6391 or contact your unit or service area.

To complete the OPOC-MHA patient or family member/supporter experience surveys online, please first contact the Quality/Patient Safety/Patient Experience office at 905.430.4055 extensions 6770, 6769, or 6391. We will provide a key (password) to online respondents. Once you receive your key you can use it to initiate your online survey at www.opoc.ca

The survey key (password) allows us to link questionnaires to the appropriate unit or service in the database. The key is NOT connected to any personal health information and individual respondents cannot be identified by the survey key in the results.
