Directory of Records

The Directory contains a list of the general classes or types of records in the custody or under the control of Ontario Shores.  Its purpose is to assist members of the public in exercising their rights of access under the Freedom of Information & Protection of Privacy Act (FIPPA) by listing and describing the hospital’s records holdings.
 

Record Category Description
Corporate Governance Records relating to the Board and its committees, minutes, policies, by-laws and corporate members list.
Administration and Program and Support Services Records relating to the operations, policies and procedures, minutes of meetings and corporate communication to stakeholders and partners. Records relating to annual reports concerning goals and strategic plans, as well as financial and other reporting data. Records relating to providing patient services and the management and delivery of clinical support services.
Finance Records relating to financial management including records of accounting, general ledgers, financial statements and tax records. Source documents including records relating to accounting transactions such as: requisitions for payment; invoices; expense claims; cheque requisitions; accountable advances; cash receipts and banking documents.
Clinical Records Records relating to patient care including diagnostic imaging, dental, pharmacy and patient register.
Research Records relating to research studies and administration of research activities.
Human Resources Records relating to management of hospital employees, physicians, students and volunteers and related administrative services.
Procurement Records relating to procurement documents (RFP, RFQ, tender), quotes/proposal/bid submissions, purchase orders, non-competitive approval form, vendor evaluations and contract management.
Capital Expenditures Records relating to the planning, construction and commissioning of new facilities and equipment.
