Ontario Shores Centre for Mental Health Sciences (Ontario Shores) partners with Homewood Health, a Canadian health organization who provides services to our employees for their Employee and Family Assistance Program (EFAP) services.
Homewood Health has recently communicated to Ontario Shores that they have been involved in a data incident which involved unauthorized access to some personal information of our employees and their family members related to the EFAP services. Homewood has taken immediate steps to contain the incident and ensure that its systems were secure.
The nature of the information that was affected
For employees and their families who have received services from Homewood Health, the following information that may have been affected includes: name, date of birth, email address or telephone number. In some cases, the records include the type of service accessed or information relating to the services provided.
Please note that no government issued ID information was affected, as it is not the practice as part of EFAP services to collect this information.
Employees concerned about status of their personal information should direct all inquiries to Homewood Health at 1-833-787-2862 or submit an inquiry using their online form.
