Ontario Shores Centre for Mental Health Sciences (Ontario Shores) revitalized its Patient Rights and Responsibilities to reflect the hospital’s mission, vision and values and support our commitment to each patient’s recovery goals in a therapeutic and safe environment.
 

Patient Rights

Ontario Shores values Respect for all individuals through the:

  • Right to be treated with respect
  • Right to dignity and independence
  • Right to effective communication, information and education
  • Right to be fully informed about all treatment options
  • Right to make an informed choice, and give informed consent to treatment

Ontario Shores values Safety for all individuals through the:

  • Right to a safe environment and freedom from harm

Ontario Shores values Excellence for all individuals through the:

  • Right to quality services that comply with healthcare standards

Ontario Shores values Community for all individuals through the:

  • Right to have support person(s) as part of treatment
  • Right to make a formal compliment or complaint

Patient Responsibilities

  • Participate in your care
  • Communicate with your health care team
  • Treat others with dignity and respect
  • Keep yourself and others safe

These Patient Rights and Responsibilities align with Ontario’s Excellent Care for All Act, as it pertains to a Patient Declaration of Values.
