REB Frequently Asked Questions

Approval is limited to one year for all research projects. Specifically, the expiry date is one year, minus one day, from the final approval date. So if your research project was approved on May 21, 2017 then the expiry date will be May 20, 2018.

Requests for annual re-approval must be made by completing an annual/continuing approval application form (see 'Applications and Forms' under Annual/Continuing Review) and providing a status report to the REB. With your submission, please submit a cover letter to the attention of the REB Chair, the Annual/Continuing Approval Checklist and Form, and the currently approved protocol and consent form(s). 4 weeks prior to study REB expiry date, please submit a hard copy of the annual renewal application to the Research & Academics Department for approval before submitting to the REB office. Once signatures are obtained, please submit the paper copy to the REB Office in 7-2029. Please also send an electronic copy of your renewal request and all supporting documents to @email.

Researchers are responsible for ensuring REB files are kept up to date and that certification is current throughout the study. The REB Coordinator will send the study PI a reminder letter to submit the annual renewal application at 60 days and at 30 days prior to the study expiry date.

All protocol changes, whether major or minor, must be submitted for approval prior to implementation. With your submission, please submit a cover letter to the attention of the REB Chair, the amendment checklist and report form (see 'Applications and Forms'), and all supporting documents, where applicable. Possible supporting documents include: an amended protocol with track changes and clean copy, amended consent form(s) with track changes and clean copies, and all other amended documents (e.g., recruitment flyers, study questionnaires, etc). Please submit a hard copy of the amendment application package to the Research & Academics Department for Research Impact Review and approval before submitting to the REB office. Once signatures are obtained, please submit the paper copy to the REB Office in 7-2029. Please also send an electronic copy of your amendment request and all supporting documents to @email.

When all study activity is completed, a Final Study Closure Report (see 'Applications and Forms') must be submitted to the REB Office with an update on study activity. Please submit the paper copy to the REB Office in 7-2029. Please also send an electronic copy of your closure report and all supporting documents, if any, to @email.
