Durham Region Psychogeriatric Resource Consultation

Durham Region Psychogeriatric Resource Consultation Program is a Ministry of Health funded outreach program designed to meet the educational needs of health care staff working in: 

  • Long-term care facilities
  • Community service agencies
  • Homecare organizations including Community Care Access Centre

The Psychogeriatric Resource Consultants (PRCs) specialize in the translation of knowledge to practice using both didactic education and case based learning. The PRCs also are responsible for the development of dementia networks in the Durham Region.

Client groups served include those working with:

  • People suffering behavioural disorders associated with a dementia or related disorders
  •  Seniors with mental illness who have significant functional limitations and require complex care
Service Access
  • Long-term care facilities
  • In house PIECES resource people
  • HCC Care Coordinators
  • Community Service Agencies managers e.g. Adult Day Program or other
  • Geriatric Mental Health Outreach Teams
