The Outpatient Complex Psychosis and Consultation Service at Ontario Shores consists of an interprofessional team that provides flexible service tailored to the individual needs of a person. As a natural extension of the specialty inpatient service we offer a range of expert services that include:
Services include:
- Assessments
- Consultations
- Behavioural support
- Shared care
- Follow-up (Ongoing support for the individuals’ recovery process and goal attainment supportive counseling, medication support, symptom management and support)
Age
For ages 18-65
Inclusion Criteria
- 18 to 65 years
- Diagnosed with a Psychotic Disorder
- Evidence of persistence of illness (present greater than 6 monhts)
- May demonstrate evidence of impaired functional ability
Exclusion Criteria
- Patient is primarily concerned with applications for ODSP/insurance/legal proceedings/CAS assessment
Service Access
Referrals are accepted from a healthcare professional, family physician or nurse practitioner. All referrals are welcome.
