I've had the privilege of serving as a Clinical Care Coordinator in the Integrated Pathway for people diagnosed with Schizophrenia. This role is incredibly important because it allows us to be the bridge between our patients, their families, and our dedicated healthcare teams.

Why is the role of a care coordinator so important? In my opinion, it's because we're working to advocate for some of the most vulnerable individuals in our community. People diagnosed with schizophrenia often face high rates of hospitalizations and decompensation in the community.

In a fragmented health care system, four groups have come together to reduce inefficiencies, prevent people from falling through the gaps, and align practitioners around a best-practice approach to care for people diagnosed with schizophrenia, their families, and their care teams.
