Ontario Shores Centre for Mental Health Sciences (Ontario Shores) works in partnership with York Region to provide an Outreach Van Service for homeless youth that locates and connects them with appropriate care in the community. A full-time mental health nurse from Ontario Shores works with a coalition of community partners to make the Outreach Van Service possible. Currently, Ontario Shores is involved with a related pilot project. A mobile metabolic clinic has been developed in response to the physical health needs of the patients addressed by the Outreach Van Service.
