Making a Request for Information

Beginning January 1, 2012, you may use the Freedom of Information and Protection of Privacy Act (FIPPA) to request access to information from hospitals within Ontario, including Ontario Shores. You can request access to a range of information held by our hospital since January 1, 2007. Access to an individual’s personal health records or personal health information, however, will continue to be governed by the Personal Health Information Protection Act (PHIPA).

Ontario Shores features a comprehensive Website containing more than 400 pages of content highlighting our organization, the services and programs we provide, research, education performance and much more. You may wish to explore the site before making a formal request for information.

Formal Request

FIPPA requests will be processed as expeditiously as possible, in accordance with FIPPA. To make a FIPPA request, please complete and submit a request form.

A $5 application fee (cheque, cash *in person only, money order) per request payable to Ontario Shores Centre for Mental Health Sciences is required for FIPPA requests.

Unless the information requested is already made available on our website, there will be additional charges related to the cost of searching for, preparing and sending the information requested. Please be very specific as to the material you are requesting. The more detail provided will assist in searching and obtaining the information and will help reduce the administration cost and additional fees.

Submitting the form:

  • Please fill out the above form and deliver or mail it to:
    Ontario Shores Centre for Mental Health Sciences, Privacy and Access Office (FIPPA Request), 700 Gordon Street, Whitby, ON L1N 5S9, along with the $5 application fee.
