Assertive Community Treatment Team (ACTT)

Two Assertive Community Treatment Teams (ACTT) are operated by Ontario Shores. ACTT Durham and ACTT Haliburton, Kawartha and Peterborough Region (HKPR) are interprofessional clinical teams that provide treatment, recovery and support services to individuals with a serious mental illness and who require a high degree of support in their community. Crisis service is available 24 hours a day, 365 days a year. The program brings mental health services to patients in the community on a long-term basis and with flexible levels of intensity. About 75 per cent of services are provided outside the office and in homes within the community. ACTT offers increased patient engagement, relationship building and individual support.

The teams consist of nurses, social workers, occupational therapists, psychiatrists, case managers and peer support workers. ACTT works with patients to support them in their journey to recovery and to integrate comprehensive services into their everyday lives.

  • Crisis management
  • Psychotherapy
  • Individual and/or family counselling
  • Health and wellness support
  • Medication awareness
  • Group therapy
  • Vocational, recreational and pastoral counselling
  • Discharge and transfer planning

For ages 18-65

Inclusion Criteria

Individuals 18 to 65 years old with a serious mental illness who require a high degree of support in their community.

Exclusion Criteria
  • Primary diagnosis of Personality Disorder
  • Substance abuse
  • Developmental disability
  • Disorders that can be serviced by other specialities
  • Risk of being violent
  • Resident of a Long Term Care Facility
Service Access

Referrals are accepted from a healthcare professional or by self referral from Durham Region, Haliburton, Kawartha Lakes Region and Peterborough.
