Dual Diganosis Outreach

The Dual Diagnosis Outreach program at Ontario Shores Centre for Mental Health Sciences (Ontario Shores) provides services to individuals 16 years old and older who are medically stable and present both an intellectual disability and a serious mental illness and/or associated challenging behaviours. Educational presentations are available upon request.

Services include:

  • Assessments
  • Consultations
  • Behavioural support
  • Shared care
  • Assistance connecting patients with community resources

Individuals must have a documented intellectual disability with a mental illness and/or challenging behaviour.

How to access this service

Referrals are accepted from a medical doctor or healthcare professional. All referrals are welcome. However, referrals from outside our primary service area are accepted for consultation only.
